The first step to creating your survey in CampaignSidekick is to draft your survey with your campaign, planning the questions you will ask and the format that you wish to ask them in.
Do you want two questions or five?
How many responses do you want for each question?
Do you want one page or four?
What question format do you want to use?
Do you want your survey to move from one page to the next, or to jump to different pages depending on the answer given to a question?
In designing your answers, it is often good to include a Refused option and an End Survey option.
Survey Fields are:
Multiple Choice: Select one and only one of the possible answers, with all answers visible on the screen
Checkbox: Select one or more of the possible answers
Dropdown: Select one of the options from a dropdown menu (this saves space and allows more questions per page)
Notice: This places a block of instruction text for the Canvasser without any recording option.
Next Button: At the bottom of each page there must be a Next Button to allow movement through the survey.
Notes: This allows for the canvasser to take notes about their interaction with a voter or record an answer not listed as one of the pre-selected Survey Options (add "Other" as an answer to a survey question and then add a notes field with the header of "If Other:"
Email: This is a field to collect the voter's email address to add it to their Voter Profile, and it also allows you to send an email to the voter if the email is collected. (links for settings/email articles)
Cell Phone: Collects the voter's cell phone number to add to their Voter Profile (Link article Dashboard/Voter Search/Voter Profile).
Home Phone: Collects the home phone for the voter's address to add to their Voter Profile(Link article Dashboard/Voter Search/Voter Profile)
Video: Allows the campaign to embed a video in the survey to play to voters at their door. This option also has the ability to email the video to the voter. (link to email setup in Settings)

2. Do you think that we should have more dog parks?
Answers: Yes,No, No but improve the parks we have, Undecided, Refused, End Survey

Answers: Yes, No, Undecided
4. There are many ways to show your support! Would you like:
Answers: Yardsign, Yardsign Delivered, Updates,Volunteer,Attend Event
Page One, Question One, Any survey answer other than End Survey goes to Page Two
Page Two Question Two, any survey answer other than End Survey goes to Page Three
Page Three Question Three, Answer Yes goes to Page Four, No and Undecided End Survey
Page Four Question 4 all answers End Survey.
